Intestinal permeability is a key mechanism linking gut to skin. When the gut barrier is compromised, bacterial endotoxins including lipopolysaccharide (LPS) enter the circulation and trigger toll-like receptor 4 (TLR4) signaling in skin keratinocytes, sebocytes, and immune cells. This TLR4 activation promotes the production of pro-inflammatory cytokines and neuropeptides in the skin, contributing to the inflammation, sebum dysregulation, and barrier disruption characteristic of acne and other inflammatory skin conditions. ## Acne and Gut Dysbiosis
The evidence linking gut health to acne is substantial and growing. A systematic review published in the Journal of the American Academy of Dermatology found that 54 percent of acne patients had significantly altered gut microbiome compositions compared to control groups — specifically, reduced Lactobacillus and Bifidobacterium species (known for their anti-inflammatory properties) and enriched Firmicutes and Proteobacteria species associated with increased intestinal permeability and inflammatory metabolite production.
A landmark 1960s study — later confirmed by modern research — gave 300 acne patients a probiotic (Lactobacillus acidophilus and Lactobacillus bulgaricus) and found that 80 percent experienced clinical improvement. Modern randomized controlled trials have confirmed these early findings: a 2016 study published in JAMA Dermatology found that a daily Lactobacillus probiotic supplement significantly reduced acne lesion count compared to placebo over 12 weeks, with the greatest benefit in inflammatory rather than comedonal acne.

High-glycemic diets — a well-established acne trigger — exert their effects partly through gut microbiome modulation. ## Eczema and Intestinal Permeability
Atopic dermatitis (eczema) has particularly strong gut-skin axis connections, well-established from birth. Research has consistently found that infants with eczema have significantly different gut microbiome compositions from birth — specifically reduced Bifidobacterium species and increased Staphylococcus and Clostridium species — compared to infants who remain eczema-free.
