    Small intestinal bacterial overgrowth (SIBO) is a condition in which excessive bacteria — often species that normally inhabit the large intestine — colonize the small intestine, where bacterial populations should be sparse. The small intestine is primarily designed for nutrient absorption rather than fermentation; it normally contains fewer than 10,000 bacteria per milliliter of luminal fluid, compared to over 100 billion per milliliter in the colon. When bacteria overgrow in the small intestine, they compete with the host for nutrients, ferment carbohydrates before they can be absorbed, produce gas and organic acids that cause symptoms, and damage the intestinal epithelium through their metabolic activities.

    SIBO is substantially more prevalent than most people — and many physicians — appreciate. Depending on the diagnostic criteria used, prevalence estimates range from 6 to 15 percent in healthy adults, rising to 30 to 50 percent in people with conditions including hypothyroidism, celiac disease, Crohn's disease, diabetes, fibromyalgia, and rosacea. Most significantly, research suggests SIBO may be present in up to 84 percent of people with irritable bowel syndrome (IBS) — a finding that transforms our understanding of IBS from a functional motility disorder to a condition with a potentially treatable microbial cause.

    Additional SIBO symptoms include: abdominal pain or discomfort, particularly in the upper abdomen; flatulence; diarrhea, constipation, or alternating between both (depending on the type of gas-producing bacteria — hydrogen-dominant SIBO tends toward diarrhea, methane-dominant SIBO toward constipation); nausea, particularly after meals; and nutrient deficiencies including vitamin B12 deficiency (bacteria consume B12 before it can be absorbed), fat-soluble vitamin deficiencies, and iron deficiency anemia. Systemic symptoms including fatigue, brain fog, joint pain, skin conditions, and mood disturbances may also accompany SIBO through mechanisms involving bacterial metabolite absorption and systemic inflammation.

    Diagnosis is most commonly made via lactulose or glucose breath testing.     The Low-FODMAP diet — developed by researchers at Monash University in Australia — restricts fermentable oligosaccharides, disaccharides, monosaccharides, and polyols: the specific types of carbohydrates that gut bacteria ferment most rapidly. In SIBO, where bacteria are present in the small intestine instead of the colon, restricting FODMAPs removes the fermentation substrate that overgrown bacteria depend on, providing dramatic symptom relief for the majority of patients.

    During the elimination phase, all high-FODMAP foods are removed. High-FODMAP foods to avoid include: fructose-rich foods (apples, pears, mangoes, honey, high-fructose corn syrup); lactose-containing dairy (milk, soft cheese, ice cream); fructan-rich foods (wheat, rye, garlic, onion, leeks, artichokes); galactooligosaccharide-rich foods (beans, lentils, chickpeas); and polyol-rich foods (stone fruits, mushrooms, cauliflower, artificial sweeteners ending in '-ol'). Permitted foods include: rice, oats, and quinoa; most proteins (meat, fish, eggs); hard cheeses and lactose-free dairy; many low-FODMAP vegetables (cucumber, bell peppers, zucchini, spinach, bok choy, carrots); berries and citrus fruits; and nuts including macadamias, walnuts, and pecans.

However, SIBO frequently recurs without addressing underlying root causes: impaired migrating motor complex activity (the gut's 'housekeeping wave' that prevents bacterial migration from colon to small intestine), low stomach acid from chronic PPI use, structural abnormalities, or hypothyroidism all predispose to SIBO recurrence. Working with a qualified gastroenterologist or functional medicine practitioner to identify and address the underlying driver is essential for lasting resolution.
