The biological mechanism of red light therapy is based on the interaction between specific wavelengths of light (typically 630–680nm red and 800–880nm near-infrared) and a mitochondrial enzyme called cytochrome c oxidase (CCO). CCO is the final acceptor in the mitochondrial electron transport chain — the molecular machine that produces ATP, the cellular energy currency. ## How It Works: Cytochrome C Oxidase Skin and anti-aging applications are among the most extensively studied uses of red light therapy. A randomized, controlled trial published in Photomedicine and Laser Surgery found that twice-weekly red light treatment produced significant improvement in skin complexion, skin tone, collagen density (measured by ultrasonography), and reduced wrinkle severity compared to control. The mechanisms include stimulation of fibroblast activity (the cells responsible for collagen production), increased ATP availability for cellular repair, and reduced inflammatory signaling. Research in the journal Lasers in Medical Science — including a meta-analysis of 22 randomized controlled trials — found that pre-exercise red light therapy significantly reduced exercise-induced muscle damage markers, delayed-onset muscle soreness, and fatigue, while improving post-exercise strength and endurance performance. ## Muscle Recovery and Performance Brain health applications are a rapidly growing area of photobiomodulation research. Near-infrared light penetrates deeper than red light and can reach brain tissue through the skull in sufficient quantities to influence neural function. A pilot study published in Photomedicine and Laser Surgery found that transcranial NIR treatment improved memory recall and improved event-related desynchronization on EEG in healthy subjects. Multiple small trials in Alzheimer's and Parkinson's patients have shown promising cognitive and motor improvements. The proposed mechanism involves mitochondrial stimulation in neurons, reduced neuroinflammation, and increased cerebral blood flow.
